Effective 2/1/04:   All submissions (New, Update, and Modification) must be made on the 2003 form. 

Effective 4/8/04:   The Principal Investigaor (PI) must complete IACUC Orientation and other application training before the IACUC will consider approving the protocol.  If funds need to be transferred from another institution and the PI is not yet at UC, the protocol may be approved but animals cannot be ordered and animal studies may not begin until all training requirements have been met.  The approval will be valid for 60 days.
