Shipping Outside the US

For sending animals outside of the USA, the following instructions and the receiving institution’s rules and requirements must be followed:

  1. Animal Health Report:
    • Contact LAMS for a copy of the latest health report for your animal colony.
    • Provide the receiving institution’s information such as fax/phone numbers, PI names, email addresses, etc. LAMS can fax the health report to the receiving institution.

  2. USDA Health Certificate Application Form:
    • Forms available online:
    • Call the USDA to verify which form is needed for your shipment (614-856-4735)
    • Contact a Veterinarian to schedule a physical exam of the animals that you are shipping
    • Fill out the USDA form and send via Fed Ex, along with a processing fee, to the address enclosed
    • PI(s) may want to include payment for the certificate to be returned via Fed Ex for faster service (regular mail can take two weeks or more)

Certificates are only valid for 30 days.

  • Do not ship until approval is granted by the receiving institution and UC LAMS
  • Make sure all of the paperwork is in order
  • Arrange for shipping payments, dates, and all other details with the receiving PI
  • Complete and submit a service request form for shipping materials and submit to LAMS at least 3 business days prior to needing
